Dr. Larry Ozowara is a board certified psychiatrist and a Qualified Medical Evaluator (QME) for the Division of Workers’ Compensation. Dr. Ozowara attended Stanford University, where he earned a Bachelor's in Psychology, Master’s in Psychology, and Doctor of Medicine. He completed his residency training in psychiatry at Columbia University. Dr. Ozowara has conducted over 100 evaluations and carries over a decade of experience in mental health.

His training in psychodynamic psychotherapy informs his appreciation of the role that historical and childhood experiences play in predisposing individuals to psychiatric illness and disability. His previous role as a consult-liaison psychiatrist has aided his diligence in extensive review of medical records and his understanding of the psychological sequelae of chronic medical conditions. His more recent experience in senior leadership roles at tech enabled healthcare startup companies has fostered an in-depth knowledge and understanding of workplace dynamics that are often a source of psychiatric complaints. QME & AME

1How quickly can I schedule an evaluation?
Most evaluations can be scheduled within 30 to 60 days, depending on the availability of the applicant, whether the evaluation is done in-person or virtually and whether an interpreter is required. For any issues obtaining an evaluation in a timely manner, please contact [email protected] and provide a list of dates and time windows. These requests will be considered on a case by case basis.
2What information do I need to provide?
The evaluation request form includes information about the applicant, attorneys, and claims adjuster. Records can be submitted at the time of the request or sent separately. All records must be accompanied by a declaration to be eligible for review.
3What types of evaluations do you perform?
Currently, requests can be made for qualified medical evaluations (QME), agreed medical evaluations (AME), independent medical review (IMR), and fitness for duty evaluations (FFDE).
4What is the payment process?
For QMEs and AMEs, an invoice will be submitted according to the most recent medical legal fee schedule, as outlined by the DWC: http://www.dir.ca.gov/dwc/. Payments should be made by check and are payable to Uzoma Behavioral Health at P.O. Box 1595 Lafayette, CA 94549.
5How do I cancel or reschedule an evaluation?
Cancellation and reschedule requests may be made here, or by emailing [email protected].
6What is the QME cancellation policy?
The QME must be canceled or rescheduled at least six (6) business days prior to the date of the evaluation to avoid incurring the late cancellation fee. Late cancellations and no shows will incur a fee of $503.75 in addition to $3 per page reviewed by the physician in excess of 200 pages, as outlined in the DWC billing guides.
7How are the evaluations conducted?
Evaluations are generally conducted via Google Meet, unless an in-person appointment is requested by the applicant. A meeting invite, including the video link, will be sent to all attendees prior to the evaluation.
8What days and times are evaluations done?
Evaluations are generally conducted on weekdays between 9:00 A.M. and 5:00 P.M. Additionally, Saturday evaluations (for virtual appointments only) are sometimes available.
9What should the applicant expect prior to the evaluation?
Reminder emails will be sent to the applicant with instructions for the day of the evaluation.
10What should the applicant expect on the day of the evaluation?
Depending on the complexity of the case, most evaluations take between 1 to 3 hours. To reduce the evaluation time, applicants are instructed to complete all paperwork prior to the face-to-face evaluation.
11When should I expect a medical legal report?
Per DWC regulations, all initial evaluation reports must be delivered within 30 days of the evaluation. However, the current average turnaround time is under 10 days. To ensure timely delivery of reports, the practice also offers electronic report submission per title 8 CCR §36.7. Consent for electronic report submission can be found here.

Depositions

1What is the fee for depositions?
The fee for depositions is $1365, which includes one hour of prep time, one hour for the deposition, and one hour for review of the deposition transcript. Each component is billed at $455 an hour.
2How and when is payment made for depositions?
The check must be received at least 2 weeks prior to the deposition. The check should be made out to
Uzoma Behavioral Health, and sent to P.O. Box 1595, Lafayette, CA 94549.
3What is the deposition cancellation policy?
The deposition must be canceled or rescheduled at least eight (8) calendar days prior to the date of the deposition to avoid incurring a penalty of $455. Failure to show for a deposition incurs the same fee.
